Latin America & The Caribbean

Emerging from this general area is a dual spectrum of under-nutrition and obesity. There have been detailed research statistics recorded for most of the countries in this region, but there remains a large gap of knowledge. Children nutritional statistics are largely limited, especially in Antigua and Barbuda, but general trends can be predicted. Five percent of children under 1 are facing under-nutrition and a 1993 Ministry of Health survey found that same percentage of mostly men facing obesity with a BMI of over 30.

Obesity comes from a cultural-economic shift in the dietary practices of the population. Obesity is not specific but can affect both genders, as a Bahamas female adolescent is more likely to be overweight in comparison with her male counterpart. In the same vein a Columbian female ages 15 – 49 have a shocking 1 in 3 chance of being overweight. As well, the rate of obesity seems to increase with age. Older generations over the age of 65 show 25% of females and 20% of men suffering from this health ailment. A 2000 study in Chile report a breakdown of population statistics as: ~12% underweight, ~22% overweight and ~26% obese. Of course, that does not mean a segment of the population is not suffering from a completely different side of the problem. Nutrition deprivation still exists in just under 10% of the Cuba population where the body is severely energy deficient.

DIET

A nations diet is often strongly tied to women’s education and health. Diet starts at the very basic level, with a foundation built on a mother’s breastfeeding knowledge. Studies support that breast-feeding is a critical component for children to gain the nutrients they need to develop healthy bodies. In Belize, the natural practice of breast-feeding is more common in the rural areas. Urban cities have to fight against the trend of bottle-feeding, that most families lack the resources to upkeep. A young child-feeding teaching program for mothers is starting to gain popularity.

This region is flummoxing between centuries old dietary practices and a new changing lifestyle made available through globalization. Traditional foods of cereal, vegetables, rice, beans are competing with the increased popularity of animal fatty foods, sugar and salt products. Meat is not foreign but has always been a part of the diet in Cuba with chicken and pigs used as traditional poultry. It is the increased consumption, alongside a downward trend of exercise that creates health risk. Balance is being threatened, where especially in Barbados there is too much national reliance on animal protein. Fat is being consumed at a higher acceleration. Over the years Chile’s diet has shifted to higher saturated milk, meat and low nutrient dense foods.

Another deficiency very dangerous to young children and pregnant women is Vitamin A deficiency. Without Vitamin A there runs a real risk of night blindness. Countries such as Antigua, Barbuda, Argentina, Bolivia and Brazil must have a national dialogue and make changes to reduce this deficiency that can run as high as 56% in some areas.

Iodine is an essential micro nutrient – a deficiency in this will cause harmful goiters in the population. Luckily national campaigns, and a widespread call to iodize all salt production have shown great strides against this problem. In Bolivia, there are 40% less cases of goiter in young students. This effort is successful but depends on continued monitoring of the salt industry.

ECONOMICS OF FOOD

Health is influenced by place – which is tied to economical security. The optimal food needed to health is not always the food available to a family. These regions rely on the food that the land is able to produce and how much a family can afford to purchase. Regions like the Bahamas show economic disparity average income differing wildly between islands. A portion of the population consists of unemployed and illegal immigrants who live on a low sustenance diet. The best environment for individuals to be healthy and tall is one where poverty does not stand in the way. In 1999 the World Bank stated that 8% of the Barbados loved below the poverty line. Similarly, in places like Guatemala there have been estimates as high as 2.8 million lives that live on less than $1 US dollar.

Poverty affects resources such as water so that a little under half of Guatemala homes do not have access to running water. Resources are not spread equally, with indigenous groups in rural areas often getting the brunt end of the stick. To combat these living situations, national organizations need to be formed and carry around sustainable assistance. An example of such a program is Chile’s Social Food Program that allows families’ access to better food. A country has to rise together to improve social conditions. Columbia has slowly made progress towards greater economical security.

Yet, some type of aid can cripple an economy. Foreign aid from other countries creates an unbalanced dependent relationship. In this case, like Bolivia, a country should augment foreign aid with building the infrastructure that will allow independent growth.

HEIGHT, DISEASE, DEFICIENCY

A significant part of height is determined by environment. This is resoundingly clear when a lack of nutrients creates STUNTING in the population. This lack of nutrients can vary between countries, but anemia is high suspect of malnutrition. Argentinean women of childbearing age are deficient in iron and often pass that on to their young children. To compare the widespread prevalence of this deficiency many global institutions carry statistical studies across the area that compare height and health. In 1990 Bolivia a study conducted found that men are more stunted than their female counterparts. This stunting was also found to be me prevalent in rural areas in the provinces of Potosí, Cochabamba and Chuquisaca.

A poorer country like Guatemala sees 1 in every 2 children have evidence of some stunting. Mexico is not far off with 1.8 million of their children under five as stunted, another 800 000 underweight and 213 000 of them chronically wasted. Wasting progresses to a point where muscle and fat tissue are severely depleted. These numbers are staggering but speak to how the lack of resources for nutrition is a compound problem crippling children for years.

Acute malnutrition is seen in various regions of Argentina, Bahamas, Belize and Brazil. The Ministry of Health Belize in 1995 recorded that children under 10 suffer from being underweight (~7%), stunting (~13%) and even wasting (~6%). These numbers seem staggering but real efforts on a national level can go towards a better health environment. A renewed 1986 study in Ecuador found the rate of stunting to have improved by 8% while a 2000 study in Cuba saw malnutrition dropping to below 5% of the children population. These numbers are promising and evidence of improvement develops. In Bolivia, children were seen to reverse the effects of earlier stunting and grow up to be healthy, normal tall adolescents.

ENVIRONMENT

The two health risks of malnutrition and obesity still exist, but in varying degrees dependent on geography and economic status. In Costa Rica, women between 15 and 19 years are either underweight or obese. Generally, obesity is centered on urban cities such as Argentina’s cities of Santa Cruz and Chubut. Due to the varying availability and variety of food, height becomes a measurable tool of difference between regions.

In Argentina the average height is higher in Buenos Aires, Rio Negro and Cordoba than Jujuy where it is the lowest at 167.5 centimeters. The Bahamas follow this difference with the Crooked Islands, the Family Islands and Acklins facing the highest malnutrition. The land itself becomes a predictor of health with Brazil’s food supply generally similar across a very geographically same area. It is the Amazon and arid northeast area of Sertão that has a different environment to support different dietary food. Further, difference in cultural entities creates very different ranges of dietary practice. The country of Belize houses different ethnic groups with ranging food practices. It is extremely difficult to account for health within countries without accounting for the nuances of each segment of the population and geography. Guatemala relies on imports as droughts floods, deforestation and soil erosion prevent high agricultural yields.

The varying regions also have varying degrees of government assistance. The Uruguay national programs are set up to assist the country. Chile’s government has focused on water, education and better sewage systems to improve health. The changing landscape of a region is open to outside influence. Developed countries offer aid along with access to the idea of globalization. This globalization brings technology and infrastructure advances to the population as much as it brings cardiovascular diseases and a sedimentary lifestyle.

Health exists in relation to many factors of changing economy, education, environment and nutrition. The measures of health, such as height, demonstrate key regional differences and the gaps that require improvement.

Nutrition In Africa

Africa is a large continent with diverse nutritional challenges and roadblocks. Development varies across countries as parts of the region face unrest, under-nutrition and battles with disease.

Diet

The landscape of Africa is similar to many developing nations – it is a study of contradictions. Regions of rural poverty are contrasted with urban centers of rapid economic growth. The living standards vary vastly not only between countries but within them. The terrain offers different resources across the continent influencing local diet. In Benin, yam is a common staple in the diet. The interior of Morocco has the potential for wheat production. A region like Cape Verdean is able to produce cereal, rice, vegetables and fish. Countries with access to a coastline rely heavily on seafood for their diet.

Globalization has introduced wealth to urban centers of each country. With access to technology, diverse farming in less than fertile soil is able to take place. Many regions have adopted rice as a crop for its versatility and strength in persevering in the toughest soil. Yet, this increased development brings its share of problems. A global marketplace introduces high fat oils and sweeteners in packaged foods that promote obesity.

Countries such as Algeria depend heavily on food imports, and the diversity of the food index is still something that needs to be improved.

In places such as Benin, the majority of livestock is dependent on imports and thus the supply is limited and not spread equally across the country.

Deficiency

Micronutrients make up the majority of nutritional deficiencies in this region. Vitamin A and Iodine deficiency exacerbate health problems until they turn into serious conditions like goiter (a swelling of the thyroid gland). The infrastructure and resources are present but not in a unified manner that can give consistent health care across the populations. Unfortunately, areas that lack this system have seen the re-emergence of diseases that were previously vaccinated against like tuberculosis. Anemia or iron deficiency is a micronutrient deficiency that affects most rural women.

Female Health

Female mothers are particularly at a high risk for chronic energy deficiency (low BMI). A lack of nutritional education leads to poor decisions in infant care. The availability of cheaper brands of formula has led to a trend for new mothers to choose to bottle-feed rather than breastfeed. This choice often puts an unnecessary economic strain on the household as babies become depended on this method of feeding. Breastfeeding delivers the best nutrition for a child, which is why some countries like Ghana have implemented teachings programs. In fact, supplementation and education programs are trying to be put in place to assist pregnant women to teach a new generation of people about health practices.

Regulatory Solutions

Micronutrient deficiencies often need a widespread regulatory system in place to combat the problem. Countries like Guinea have answered this need by implementing a salt iodization program against goiter. Health infrastructure is being built, but specialized medicine is still not available in rural centers. Countries like Ethiopia have vitamin A supplementation programs, and vaccines are being given to combat preventable diseases.

A key in the fight for health is naturally fortified supplementation. Cotton oil and agricultural crops like wheat flour can be fortified with iron, folic acid and B vitamins to raise nutritional health for vulnerable segments of the population. Urban centers are entering a new phase of development that is bringing challenges to the health center that have not been previously seen. The exposure to unhealthy food options such as street foods, snacks and sugar-rich food have led to an emerging issue of obesity. Combined with a lack of physical activity, cardiovascular diseases and diabetes are straining health resources.

Water is crucial for human survival. The sanitation of water in countries like Chad is detrimental to the health of the population. Certain regions do not have access to local wells and so woman must often make long journeys across the land for cooking water.

Health and Height Conditions

Whiles genes provide the blueprint for height, malnutrition stands in the way of it being achieved. Up to 20% of children in one region suffer from STUNTING. This reduced growth rate in human development often afflicts younger children. A 2008 study in Benin saw an incredible 40% of children under 5 as stunted in growth. The region of Chad has very similar statistics, but also found that 9% of children suffered obesity on the other end of the health spectrum. Nutritional education, infrastructure and resources are needed to put development back on track.

Environment and Unrest

A major roadblock in health advances is the changing landscape brought on by civil war, disease epidemics, and environmental disasters. Civil war from 1977 to 1992 in Mozambique severely handicapped health progress by destroying agricultural production and separating families. Since the end of the civil way, the region has started the road to recovery. Similarly in Chad, shifting alliances and political changes create an unstable region where the migration of people changes according to safety. Certain regions have an influx of refuges at their border as neighboring countries are at war.

The HIV/AIDS epidemic is a prominent concern as treatments and education on the disease are lacking in certain regions. Malaria is another cause of sickness and death, specifically for vulnerable children. Kenya is actively working towards higher immunization rates but much progress is needed.

The proper utilization of natural resources is necessary for countries to have the economic ability for change. The land can provide diverse forms of capital. Chad has recently discovered oil production, and Zambia’s copper mining industry is key to their economy. What stands in the way of progress are natural disasters like the 2001-2002 drought in Malawi that led to famine across the population. Kenya has faced alternating flood and drought concerns. Some regions like Cape Verde are limited in the resources they have. The land that is farmable is limited and often lacks the necessary rain for growth.

Africa is a region of difference, and so different health policies must be made to meet the needs of the population. Health advances have been made but must further continue to improve nutrition and quality of life.

- Diet

Dietary choice is depended on availability and the geography of the region. Egypt and Iran enjoy cereal as a main staple. The more urbanized the population density, like in the Province on Tehran, the rate of animal produce increases. Nutritional balance promotes health – Jordan has collectively tried to branch out and include a meal of wheat, rice, vegetables, and animal products.

Economy is a roadblock for health advances. When large segments of the population are living below the poverty line, they are unable to eat nourishing meals. The conversation shifts to survival, instead of health. Egypt has a segmented population with different social classes getting unequal shares of food resources. Iran also suffers from having no health services for its people. Shockingly, the majority of people in Sudan live in poverty with no food security.

- Nutritional Deficiency:

The types of nutritional deficiencies seen in this region mainly include anemia, iodine and Vitamin A with smaller cases of Vitamin D, folate and B12. There lacks clear studies done on nutrition for some parts but it is seen that iron deficiency has more devastating effects on children and women. Up to 25% of that demographic suffers from anemia. Countries like Egypt have promoted supplementation and salt iodization programs to combat the rising statistics of goiter in the population. Widespread intervention along with education programs is needed to promote healthy dietary practices.

- Woman’s Health:

Women’s health is an issue that countries need to address. Iranian women suffer from anemia leading to gastrointestinal bleeding, short birth intervals and occasional infectious disease. Fortunately in Bahrain, women largely continue to breast feed so their infants are able to get nutrient dense milk. To continue this trend and offset the emerging global bottle-feeding pattern, more education needs to be done. In rural areas, breast milk is often the only source of food for children. Women prolong breastfeeding for as long as possible, meaning non-milk food gets introduced to the child late. This contributes to developmental delays and shorter growth height when compared to their healthier peers.

- Height and Health:

A common sign of under-nutrition is the prevalence of STUNTING in children. Stunting refers to developmental growth delays that can show up as insufficient height. This is a widespread concern with 1/5 Iranian child under 5 and 1/10 preschool children in Palestine qualified as stunted. Even within the country, geographical differences account for different statistics of growth. The southern end of Iran has higher malnutrition than the north end.

People are often surprised when they visit this region as the rate of development in urban centers has produced high-rise buildings in some areas. While large parts of the country deal with malnutrition, the other side of the spectrum of health concerns does exist. Tehran has a rising problem of female body concerns, with young women under-eating to be thin. In comparison, 13% of Tehran women are obese. This comes from urban globalization that has seen the influx of high fat, sugar food into cities. A lack of exercise contributes to this problem coupled with the lower prices of fast food available. Kuwait is representative of this problem, with up to 80% of women being overweight. This leads to a burden on existing health infrastructures with the rise of diabetes, high blood pressure and cancers.

- Land Resources:

Often the need for varied dietary food is outstripped the available land for food production. Bahrain’s land is not able to support much agriculture production forcing the region to rely on imports. Further, Jordan and Palestine is dry with a lack of water resources. With such a high dependence on food imports, the availability of food is tied to fluctuating market prices. To combat this external dependency, more sustained practices of irrigation need to be adopted.

Sometimes a lack of agricultural production can be replaced by another high capital economy. Kuwait has a petroleum-based economy that provides enough resources to import the majority of their food from outside their borders. Other countries with access to the coastline have seafood has a resource.

- Regulatory Changes:

Intervention is needed at a federal level to enact changes into heath across country. Bahrain has taken a step towards this direction with free medical care. Kuwait has free education, health care and immunization.

- The Shifting Landscape:

This region takes great strides in health advances but is often setback greatly with the shifting political and religious landscape. Until the beginning of the nineties civil conflict across Lebanon has wrecked the region. More recently, Palestine has had a second Intifada that devastates social structures and preys on the vulnerable segments of the population – children and women. School enrollment is threatened by violence. Countries with tribal leaders all competing for control have left destroyed buildings and regressed regions back in development. It is more devastating when the conflict coincides with natural disasters like floods in Sudan. Civil unrest creates great population migration and refuges that stress a system that is unable to meet the needs of its citizens.
